Volunteer Commitment Form
I agree to abide by the following:
I will sign in at the main office upon arrival and sign out when I leave for the day.
I will wear my Volunteer Badge at all times while participating in volunteer activities.
Except in case of emergency, I will give 24 hours notice when I cannot keep a scheduled assignment.
I will follow the dress code of the program where I am working.
I will only use the adult bathroom facilities.
I will never be alone with an individual student unless supervised by a teacher or school staff.
I will not contact students/clients outside of school hours, or exchange contact information.
I understand that as a volunteer, I am considered a mandated reporter, which means I am required by law to report all known or suspected cases of child abuse or neglect. It is not my job to investigate or make a determination if the allegations are valid. If I reasonably suspect child abuse or if a pupil shares information with me that leads me to believe abuse or neglect has taken place, I will immediately contact my supervisor.
I will treat all students, families, and employees with respect regardless of their race, gender, religion, sexual orientation, gender identity, disability or immigration status.
I will not share confidential information about a student with anyone inside or outside of the school.
I will report behavior problems to the teacher or other supervising school personnel.
I will respect the authority of all school personnel.
I will learn the rules regarding drills and emergencies, and follow my supervisor’s directions.
My time spent providing volunteer services is not compensable, and I have freely chosen to donate my time without compensation, remuneration, or offer of employment.
